This is a separate tool from Breathing Space — and one that can directly change how your creditors treat you.

The Debt and Mental Health Evidence Form is a document your healthcare professional completes that informs creditors about how your mental health condition affects your ability to manage your finances. Once a creditor receives a completed DMHEF, they are expected under FCA guidance to treat you with appropriate sensitivity — which can mean pausing collection activity, removing pressure tactics, setting up reduced payment arrangements, or waiving certain fees.

Following a review by the Department of Health and Social Care, the British Medical Association, and the Money and Mental Health Policy Institute, the form has been updated — it is now shorter, and GPs who agree to complete it can no longer charge for doing so. The FCA’s Consumer Duty rules, which came into force in 2023 and apply fully now, require regulated lenders and creditors to act in the best interests of customers in vulnerable circumstances. Mental health conditions are explicitly included in the FCA’s definition of vulnerability.

Depending on your situation, several routes may be available:

Debt Relief Order (DRO): If you have less than £50,000 of debt, under £2,000 in assets, and £75 or less per month in spare income, a DRO can legally write off qualifying debts after 12 months. How to talk about debt when your mental health is suffering

This is the part most debt guides skip entirely. Knowing what you’re entitled to means nothing if the thought of making a phone call sends you into a panic spiral.

A few things that actually help:
